What documentation do I need?

Acceptable documentation must be obtained from a certified health care professional who is trained and qualified to diagnose the conditions for which the accommodation is being requested. Examples of proper healthcare professionals include the following: 

  • Certified audiologist
  • Certified school psychologist (CVASP registered if in BC)
  • Neurologist
  • Neuropsychologist
  • Ophthalmologist
  • Physician or medical specialist
  • Psychiatrist
  • Registered clinical psychologist 
     

If you have a learning disability, we need a psychological-educational assessment from a registered clinical or school psychologist. This assessment should explain how the disability affects you and suggest what services would help.

What if I don’t have any documentation? 

Students unable to provide documentation or who have outdated documentation are still encouraged to complete the After completing the self-assessment, a disability access coordinator will help you determine the next steps.  

How long is documentation valid for? 

For diagnosis of a learning disability, a psychological-educational assessment must have been completed in the last five years, or the assessment must have been made after age 18. 

All documentation should: 

  • Must be dated within the last five years
  • Outline the nature and extent of the disability
  • Explain how the disability impacts the student in an educational environment
  • Outline the functional capacity in an academic setting
  • Provide recommendations for supports that help manage the disability in an educational setting  

For diagnosis of disabilities other than learning disabilities (mental health, ADHD, autism etc.), all documentation must be current within the last three years.   

All documentation should: 

  • Be within the last three years
  • Outline the nature and extent of the disability
  • Explain how the disability impacts the student in an educational environment
  • Outline the functional capacity in an academic setting
  • Provide recommendations for supports that help manage the disability in an educational setting
